This form allows users in the United States to opt out of binding arbitration for disputes related to the NBCUniversal Services. If you wish to opt out of the arbitration agreement in our Terms of Service (TOS) (full terms available

), you must complete and submit the form below within 30 days of first using the NBCUniversal Services or otherwise agreeing to the TOS. If you opt out of the arbitration agreement in our TOS by submitting this form, all other parts of the TOS will still apply to you. So will any other arbitration agreements that you have entered with NBCUniversal, or that you may enter with us in the future. If you wish to continue using binding arbitration for disputes related to the NBCUniversal Services, or if you are located outside of the United States, no further action is necessary.
